PDF Forms are defined in the PDF Forms section of the UA Administration Application. When defining a PDF Form a name, PDF Form and language must be specified. The agency can add a version of the form for each language (locale) configured for UA. Once defined, these forms are accessible from the Print and Mail Form page of UA.
